New patient registration is easy

If you're ready to register, click on your state below to book your appointment.  You will receive a text to start the registration process, this only takes about 10 minutes. The form will require you to fill out the information about your current symptoms, medical history, and medications. Once you have completed your intake and signed the consent forms you're all set!  You will receive a same day reminder link for your appointment.
