Weight Loss Program

Our platform was designed with you in mind.  We worked hard to put together a program that would work and we designed it with some of our own employees as participants.  When we came together to brainstorm, the emotional terror that lives everywhere was in our very own boardroom.  Obesity and overweight health concerns can be emotional and emotions were running high as we grew to perfecting Embodify Me.  The goal to create a way for our members to feel embodied as we did by having a competitive mindset and personal experience, makes our program made from the heart.  We know our program works, because we experienced it first hand. 

Here's how are program works. 

• Complete the registration process
• Schedule your appointment
• Visit with your provider
• Review your symptoms
• Comprehensive review of any current medical conditions
• Go over past medical history
• Discuss your goals
• Create your medication and treatment plan
• Develop a healthy eating habits and exercise plan
• Create your "Passage" follow up) schedule

How does the "Passage" appointment work and what is it? 

• A "Passage" appointment is considered a follow up. 
• "Passage" appointments are done every 30 days
• You must be consistent with your "Passage" appointments to ensure no delays in your medication refills. 
• You will receive an email link to fill out our "Passage Form" (The "Passage" form requires you to answer questions on the previous 30 days in regards to your compliance with your medication plan, diet and exercise regimen.  
• Once you have completed and submitted the "Passage Form" your provider will review the document, ensure all questions were answered and complete the "Passage" appointment.  
• Upon completion of the "Passage" appointment your prescription will be sent in on the same day to your preferred pharmacy.
