Payment and Refund Policy

We are committed to providing you with a quality weight loss program.  Our Patient Financial Policy is important to our professional relationship. Please understand that payment for services is a part of that relationship. If you have any questions about our fees, our policies, or your responsibilities, please do not hesitate to ask. It is your responsibility to notify our office if any patient  information changes (i.e., name, address, telephone, payment information, etc.) 

Our payment policy requires each patient to have a method of payment on file.  Payment is due at the time of booking your appointment.  You may cancel 24 hours in advance for a full refund.  If you cancel within the 24 hours you will be charged a $50.00 late cancellation fee.  A no show to your appointment will also result in a $50.00 fee.  If you are experiencing difficulty in any way you must contact our office to discuss your options.  

Payment policy for the Hormone Replacement Therapy Program requires payment in full at the time of booking.  This allows us to prepare for your appointment and have the proper tests ready to be sent to you.  Cancellation for this program requires 72 hours in advance.  If you cancel within the 72 hours, your payment will be held as a deposit to recover your appointment at a later date.  There are no refunds.  

Special circumstances involving where the patient is not a good candidate for the weight loss program, will result in a refund, should the provider clinically determine the findings for the patient.  If a patient is dissatisfied with a service or treatment provider by a provider, please contact our office immediately to remedy the concern.  We will do everything we can to accommodate your needs and care.
