Yes, our medical doctors, nurse practitioners, and physician assistants are all board certified and licensed to treat patients.  Specializing in obesity, weight loss, substance abuse, and general health. 

We offer two types of weight loss program.  

Non-Semaglutide :

Your initial visit is $200.00 for the assessment and review of symptoms. 

30-day follow-ups and refills are $150.00.  (30-day follow-ups are required for this program).  

Semaglutide Program:  

Charges are not automatic and are charged on the day of the visit with your provider.  Prescriptions are sent on the same day of your appointment to your preferred pharmacy on file. 

No, we are not a membership service.  There is no contract or obligation to continue treatments with our office.  You may discontinue your treatment at any time by calling or emailing us.  We discourage the hard stop of any medications, and it is best to communicate with your provider so that proper steps can be met to discontinue treatment.

We offer a variety of weight loss-assisting prescription medications.  Phentermine, Topiramate, Semaglutide, Naltrexone Bupropion, Loamaira Orlistat, and more.  Some medications are combined with others, such as Hydrochlorothiazide and Prozac, to help maintain your weight loss goals.  The combination can help with side effects that may be experienced when taking a new medication.

No, you are responsible for the cost of medication. When you receive your prescription, you may use your insurance. We cannot guarantee your insurance will cover prescriptions, and do not do prior authorizations. If insurance does not cover the cost of your prescription, we recommend using Good Rx, Rx Saver, or other discount services. The cost of the medication is typically between $10-$25.

For patients on the Semaglutide Program, medication is included in the cost of your package.  

We have offices across the United States.  Our two main offices are locted in Texas and Florida.  Although, most of our visits are performed via our HIPAA compliant tele-health platform.  You will receive a link to join your virtual visit with your provider 10 minutes prior to your appointment.  It is important that you have everything required for the appointment to assure there are no delays in starting your program.  If you need an in person visit, for receiving weight loss injections, contact us for availability.

No, we do not accept insurance.  Your payment method must be on file, you will be charged after you have submitted your intake registration form on the day of your visit. Your first appointment will be confirmed on the same day or within 1 hour.  We offer same day visits with a one hour cushion.  You may choose to use your insurance for your prescription that is sent in by your provider.  We cannot guarantee that your insurance will cover the prescription, and encourage you to use Good Rx or other discounted Rx service.

In order for you to lose weight, create a new, healthy mindset and keep the weight off, we'll help you discover what dietary measures will work for you and your lifestyle!  It is important to remember that food intake and proper portioning is important to losing weight.

Risks are minimal, however we do a full medical assessement of your current and past medical history to ensure you will benefit from our weight loss program.  Educational material will be provided for you, as well as support during your time in the program. 

Our payment policy is simple, you pay on the day of your appointment with your medical provider, and payment is due at the time of the appointment.  No refunds are permitted under any circumstance.  You are provided a medical treatment by a licensed medical provider.  Should you be unhappy with your visit, or treatment method, it is important that you contact our office immediately to remedy the concern.  If a patients payment has been reversed due to a dispute, a full investigation along with supporting documents will be submitted to the processing company and any and all fees including a $50.00 fee will be due immediately.  If you have a dispute, please contact us at support@embodifyme.com
